Upon enrollment you will be assigned a Care Manager. Generally, all of our services include Care Management especially if you require an array of services. Our Care Managers are licensed social workers who have experience in working with individuals with mental health, substance use, and/or developmental disorders and with navigating local providers and resources available.
When receiving treatment for any type of disorder, it often requires an interdisciplinary team of providers. Unfortunately, providers don’t always communicate with one another which can have a negative impact on your continuity of care. Services are duplicated or even missed altogether. In an effort to centralize your care, we serve as an Advocate, Coordinator and Consultant for clients and their families in all areas including: treatment planning, clinical providers, insurance companies, the legal system, medication management, and crisis intervention.
Our team follows a client-centered approach. This means YOU are the expert in your own life, and we are to help you make informed choices. Additionally we offer supportive counseling to address issues and barriers toward achieving mental and physical wellness. Regardless of your diagnosis, we at Lotus Care believe in the importance of self care for everyone and learning the skills needed to do just that.
